 Provide immunization records--NO EXCEPTIONS!! You may have the prior school fax them to 253-530-4420. No shot records, no registration! |
 Provide a copy of the student's birth certificate. |
| Provide something with your physical address and name on it. ie: utility bill or mortgage agreement. |
| If your student will be in 10th, 11th or 12th grade, the counselor will need an unofficial copy of their transcript to make sure we are getting them into the right classes. |
| Provide the address of the prior school if outside of Washington. |
| If your student receives Special Education services, the school will need to have a copy of their current IEP and most recent psych-educational evaluation prior to registration. |
